KeHo Home Care provides support for Medicaid-Eligible individuals and their families. You may be eligible for pay and support in Indiana if the person you are caring for is:
1. 18 Years or Older
2. Needs Medical or General Assistance with daily activities like eating, bathing, and getting dressed.
3. Eligible for Medicaid
4. Live in the same home as you
5.Related to you
In Indiana, Medicaid pays for home care through waiver programs such as Pathways, Health & Wellness, and/or TBI Provider Waiver.
Caregiver Coaching and Behavior Management (CCBM)
Available under Pathways and Health and Wellness What is CCBM? CCBM provides coaching and emotional support to family caregivers. CCBM will equip the individual’s caregiver with necessary skills to manage the individual’s chronic medical conditions and associated behavioral health needs related to cognitive impairment and/or dementia. CCBM will conduct an initial consultation for assessment of the caregiver to determine needs and follow up with a home visit, virtual visit, or telephonically
Structured Family Caregiving
includes the following activities (Levels 1-3): • Structured Family Caregiving includes (Levels 1-3) • Services provided by a principal caregiver who is the spouse of the participant (legally responsible individual) • Home and community assistance care services related to needed IADLs • Attendant care services related to needed ADLs • Medication oversight (to the extent permitted under state law) • Escorting to necessary appointments, whenever possible, such as transporting individuals to doctor (Note: When provided, such transportation is incidental and not duplicative of any other Indiana Medicaid State Plan or waiver service.) • Appointments and community activities that are therapeutic in nature or assists with maintaining natural supports • Other appropriate supports as described in the individual’s service plan
Structured Family Caregiving emphasizes the participant’s independence in a setting that protects and encourages the participant’s dignity, choice, and decision-making while preserving self-respect.
Structured Family Caregiving
Structured Family Caregiving means a caregiving arrangement in which a participant lives with a principal caregiver who provides daily care and support to the participant based on the participant’s daily care needs. The principal caregiver may be a nonfamily member or a family member who lives with the participant in the private home of the participant or the principal caregiver. Necessary support services are provided by the principal caregiver (family caregiver) as part of Structured Family Caregiving service. Caregivers must be qualified to meet all federal and state regulatory guidelines.
